To apply for your U.S. passport, you must:

Submit your completed application, DS-11, to request the Passport Book, Passport Card, or both; or submit the Renewal form, DS-82, to renew your passport. The application forms DS-11 and DS-82 can be downloaded from this website: travel.state.gov.
Submit your original evidence of U.S. Citizenship:
- A certified copy of your birth certificate;
- An original Certificate of Naturalization or Citizenship;
- An original Report of Birth Abroad; or
- A previously issued, undamaged U.S. passport.
Bring a photocopy of your evidence of U.S. Citizenship.
Submit one recent Passport Photo - 2 inches x 2 inches. No glasses in photos.
Present valid Photo Identification - Driver’s License, State ID Card, current passport, or Military ID, and submit a photocopy of the front and back of that ID.
Payment of passport fee by credit/debit card, check, and/or money order.
U.S. Passport Book: Used for international travel by land, air, and sea.
U.S. Passport Card: Used for entering the United States at land border crossings and sea ports-of-entry from Canada, Mexico, the Caribbean, and Bermuda. Not valid for international air travel.
Validity of Books and Cards: 10 years for adults (ages 16 and older); 5 years for minors (under the age of 16).
Processing Times by mail: Routine: 4-6 weeks; Expedited: 2-3 weeks and an extra $60 fee.
Passport applicants must appear in person, including minors. Passport applications for minors under 16 require the consent of both parents or evidence that one parent has sole legal custody.
